Pulmonary vein thrombosis (PVT) is an uncommon disease with a very similar clinical presentation as pulmonary artery embolism. We present a case of a 53 year old female patient who presented with chest pain and shortness of breath and was later found to have pulmonary vein thrombosis. The diagnoses was established with a CT scan of chest which showed a filling defect in a right segmental pulmonary vein. Patient was started on heparin drip and later discharged on oral apixiban. Three month later, CT scan showed resolution of thrombus. PVT presents with non-specific symptoms which can be easily mistaken for pulmonary artery embolism. While investigating a patient for cryptogenic stroke, diagnoses of PVT should always be considered. Diagnoses is made via CT pulmonary angiogram or magnetic resonance angiography. Echocardiography also aids in confirming a diagnosis. Treatment is similar to pulmonary arterial embolism with systemic anticoagulants.

Purpose: To briefly discuss a young, otherwise healthy Caucasian male who presented to an emergency department with the complaint of acute scotoma. Observations: External ocular and anterior segment examination were unremarkable. A dilated fundus examination of the right eye showed a wedge-shaped area of retinal whitening and cotton wool spots in the inferior macula between the optic nerve head and fovea. Conclusions and Importance: After a thorough work up including a limited coagulopathy screening and rheumatologic and infectious panels, it was determined the most likely cause of presentation was systemic Bartonella infection contracted from the patient’s pet cat. The patient was treated with oral doxycycline 100 mg BID for 4 weeks and maintained stable vision on subsequent follow ups with no recurrent arterial occlusions. We hope this report provides clinicians with an insight into an uncommon ophthalmologic presentation of Bartonella henselae infection, as well as a reasonable template for successful treatment if such a case is encountered.

This manuscript attempts to comparatively decipher the acroamaticsubstances from Platonism and Confucianism, followed by amalgamation of their intrinsic reciprocity and heterogeneity, and then seeks to theorise international development issues that may be theoretically addressed on the grounds of both international relations theory and theoretical merits of Platonism and Confucianism. Both Platonism and Confucianism, which could be the indirect philosophical ramifications of a set of colloquies among myriad magnificent interlocutors within their respective historical periods, unveil substantial influences upon, and significant implications for, the Occidental Civilisation and Chinese Civilisation, ranging from epistemological interpretations of power, justice, knowledge and even governance systems. It is fair to argue that those intellectual notions from Platonism and Confucianism generate indelible merits and understandable blemishes owning to the historical circumstances, socioeconomic development, and technological advancement in a far more embryonic stage. If both can be put on an axis of international studies theory, their legacies, whose significance might outweigh their ideational blemishes, can function as the alternative theoretical guidelines to international human development issues. To evaluate the applicability and their merits on an axis of international studies, this manuscript conducts comparative studies of global poverty alleviation with the Platonic-and-Confucian-generating spirit of the UN Sustainable Development Goals, of climate catastrophe and COVID-19, and ultimately seeks to dialectically reconfigures the ideational interplay of Platonism and Confucianism with international studies, which generates formulas. Based on multiple research, it could be so far concluded and expected that irrespective of their legacies and limitations, it literally behooves analysts and connoisseurs in international studies to multidimensionally cogitate and reactivate their intellectual conceptions, especially part of constructive epistemological substances that might remain theoretically applicable to maximum inter-territorial exchanges of divergent civilisations and seeking a normative theoretical formula of addressing global development issues in a shared 21st century.

Heart failure is a complex clinical syndrome in which structural or functional cardiac disorders result in an impaired cardiac function causing symptoms that vary according to the underlying pathology of heart failure. Patients with heart failure are often challenged by the loss of independence and compromised functional abilities associated with the symptoms. This is why comprehensive cardiac rehabilitation including supervised exercise is advised. Indeed, the favorable effects of exercise training in chronic heart failure are widely recognized and exercise training is recommended in international guidelines. However, the application early exercise intervention in patients hospitalized for acute decompensation or acute worsening in cardiac function has not been sufficiently explored and, as a result, knowledge about the effects of exercise training in the inpatient setting of acute HF remained limited regardless of the research efforts made in this area. For this reason, this work aimed at reviewing and collecting previous literature to support the effectiveness and the safety of early mobilization in this patient group.

Introduction: The COVID-19 pandemic leading to National lockdowns, disruption of health care services and fear of attending health facilities has affected the well-being of the pregnant women and their babies. A very few studies done on pregnancies complicated with COVID-19 suggest that the pregnant women may present with severe manifestations leading to feto-maternal compromise. Methodology: We did a retrospective descriptive study in GMC, Kathua after attaining approval from Institutional Ethical Committee. All the pregnant women , with laboratory confirmed positive result of either RTPCR or RAT report from nasopharyngeal or oropharyngeal swab (8) from 1st of March to 31st of May 2021 were included in the study. Data was analyzed as mean, percentages and range. Results: Among the 23 observed women, the mean age was 27. 3 patients were asymptomatic, fever was the most common symptom among symptomatic (55%). X-Ray and D-dimer was done only in 2 patients with clinically moderate and severe covid 19 cases. Nearly 56% of the patients had mild transaminitis. Only 1 patient had severe illness and stayed in ICU for 8 days. Average hospital stay of women was 6 days. Preterm deliveries happened in 1 woman. 1 IUD was reported and 17% of neonates were admitted in NICU. Conclusion: All suspected pregnant women should be systematically screened, monitored and followed up. Pregnant women may present with mild disease. Neuraxial anesthesia is the better choice of anesthesia in Caesarean sections whenever possible. Perinatal morbidity may increase due to underlying maternal infection. Further investigations and follow-up studies of pregnant mothers infected by COVID-19 are warranted.
